MoDerm Membership Terms and Conditions

1. Membership Commitment & Billing

Annual Commitment: All MoDerm memberships (The Essential Radiance and The Holistic Elite) require a minimum twelve (12) month commitment.
Monthly Billing: Membership fees ($99 or $229 plus applicable taxes) will be automatically billed on the same day each month to the credit card provided at enrollment.
Early Cancellation: If a member wishes to cancel prior to the end of the 12-month term, they will be responsible for a cancellation fee equivalent to 50% of the remaining months' dues or the full retail value of all complementary services utilized to date, whichever is greater.

Service Allocation: Complementary sessions (e.g., Microneedling, Chemical Peels, HydraFacial, IV Therapy, and Skin Tightening) are allocated for the membership year.
Usage Period: All complementary services and manicures must be used within the 12-month membership cycle. Unused services do not "roll over" into a subsequent year and are forfeited if not booked prior to the membership anniversary.

Bankable Botox Credits: The $100 Monthly Botox Credit (exclusive to the Holistic Elite tier) is "bankable" and stackable. These credits accrue monthly and can be applied toward the purchase of Neuromodulator treatments at MoDerm.
Expiration: Banked Botox credits remain active as long as the membership is in good standing. Should the membership be terminated, all accrued credits must be used within 30 days of the final billing date or they will be forfeited.

Non-Transferability: Memberships, included services, and accrued credits are strictly non-transferable and may only be used by the primary account holder.
No Combinations: Membership discounts (10% or 20% on Laser and Vivier products) and credits cannot be combined with other promotional offers, seasonal sales, or flash discounts unless explicitly stated in writing by MoDerm management.
Product Availability: Discounts on Vivier products are subject to on-site availability.

No-Show & Late Cancellation: All members are subject to MoDerm’s standard 24-hour cancellation policy. Failure to provide 24 hours' notice for any "Free" or "Complementary" service appointment may result in that service being forfeited from the annual allocation.
Partner Services: Services provided by membership partners must be scheduled in advance and are subject to the specialist's availability.

Right to Amend: MoDerm Laser & Aesthetics reserves the right to modify membership pricing, service inclusions, or terms with 30 days’ written notice to the member.
